Always verify that the provider you choose is participating in our network before you receive services. While it is our goal to provide accurate information, our provider network is subject to change without notice. Providers in all areas of our service region are included in the provider search. Please be sure to select the correct product (PPO, etc.) when beginning the provider search.

This plan is sold to commercial employer groups and many FEHB Employees. Coventry Dental members do NOT have a Dental code listed on their Altius ID card. Instead, you should have recieved a separate Coventry Dental ID card that has the DenteMax logo listed on the back.

Please Note: Associated Food Store Members and Union Members do not have Dental benefits.

If you are a Federal (FEHB) Employee or other group, you may be a Monarch Dental member. If you see the word "Dental" beneath your group number on your Altius ID card, you belong to the Monarch Dental network.

Rural Health Care Notice

You may be entitled to coverage for health care services from the following non-participating providers if you live or reside within 30 paved road miles of the listed providers, or if you live or reside in closer proximity to the listed providers than to Altius participating providers:

• Green Valley/River Clinic, Green River, Emery/Grand Counties, Utah
• Halchita Clinic, San Juan County, Utah
• Monument Valley Health Center, Monument Valley, Utah

This list may change periodically. Please check our website or call for verification. Please be advised that if you choose a non-participating provider you will be responsible for any charges not covered by your Altius benefit plan.

If you have questions concerning your rights to see a provider on this list, you may contact Customer Service at (801) 323-6200 or 1-800-377-4161. If we do not resolve your problem, you may contact the Office of Consumer Health Assistance in the Utah Insurance Department toll free at 1-866-350-6242.

Out-of-Area Providers: Peak Plus Members

If you are a Peak Plus member living outside the Altius Health Plans service area, click the Search for a Provider Online link above, then select "Coventry National Network" under the product drop down field.

Out-of-Area Providers: HMO Members

For all HMO and POS members living within the Altius Health Plans service area, Coventry Health Care National Network providers are available to you when you require emergency care outside the Altius network.
